To help save you some time at Graysville Gas and Water, please find the form you need in the list below. Click on the link and then you will be able to print it out. You may either bring the forms into the office, fax them to us at 205-674-5646, or email them to [email protected] with the form name in the subject line.

If you will be returning the completed contract by email or fax, it must be notarized.
If you are sending the contract to Graysville Gas and Water by someone else other than whose name is on the form, it will have to be notarized as well.

Please note, ALL forms must be completed and signed by the service address owner or renter. Any partial request/incomplete paperwork will be shredded after 30 days.
